New Patient Information

New patients are welcome. We offer convenient office hours and participate with most insurance plans. 

All new patients are asked to complete the medical history and patient registration forms, and must present their insurance card(s) upon their first visit with our office. It is also the patient’s responsibility to bring their referral, if required, with them to our office. We ask that all patients arrive a minimum of 15 minutes prior to their scheduled appointment in order to complete the registration process.

Patients who have already completed the registration forms need only arrive 10 minutes prior to their scheduled appointment time.

Returning patients do not have to complete the patient information forms if they have been seen within the last six months and completed the process within that time.

If you are unable to keep your appointment, please notify the office within 24 hours. If, for some reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible.

Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.
